Charitable Donation Request

Portraits by Suzanne is pleased to make donations to qualifying non-profit organizations. Please answer the following questions and submit the request as early as possible prior to a function. Upon acceptance, arrangements will be made for a sample portrait and gift certificate to be delivered to you for display purposes. Minimum bid must be set at no less than 50% of the gift certificate value. Following the event, arrangements should be made for the return of the sample portrait and the winning bidder’s information registered with Portraits by Suzanne to validate the gift certificate.
